Court Street, Franklin IN 46131 July 3, 2008 RIGHT TO KNOW – PUBLIC RECORDS REQUEST #70308-Vndr To whom it may concern, Pursuant to the state open records law, Ind. Code Secs. 5-14-3-1 to 5-14-5-12, I write to request a copy of the following records: Item 1: All correspondence, memoranda, and documents pertaining to ATM Diagnostics, Dalton Consulting, and Election Systems & Software from June 1 through July 3, 2008. This should include proposals, bids etc. Item 2: A copy of all invoices for ballot layout, voting machine maintenance, and election-related project management from Jan. 1, 2006 through July 3, 2008. Item 3: A copy of Johnson County's flood insurance policy. Item 4: A copy of any claims filed with insurance or government disaster relief agencies related to election equipment. Item 5: As I have been unable to locate the names of the Johnson County Elections Board, please provide a copy of the name of each member of the elections board. Item 6: I request a copy of the documents usually called personal financial disclosure documents (or conflict of interest statements, asset disclosure statements, ethics statements, etc) for the following individuals: Jill Jackson, each member of the Johnson County Election Board, and each member of the Johnson County Board of Commissioners. Please provide documents in electronic form if they exist in that format. If documents do not exist in electronic form, please provide paper copies. If your agency does not maintain these public records, please let me know who does and include the proper custodian’s name and address. I agree to pay any reasonable copying and postage fees of not more than $100 for this information. If the cost would be greater than this amount, please notify me. Please provide a receipt indicating the charges for each document. As provided by the open records law, I will expect your response within seven (7) business days. Please be advised that violation of the open records law can result in the award of reasonable attorney fees, court costs and other reasonable expenses of litigation. If you choose to deny this request, please provide a written explanation for the denial including a reference to the specific statutory exemption(s) upon which you rely. Also, please provide all segregable portions of otherwise exempt material. Government is the servant of the people, and not the master of them. The people, in delegating authority, do not give their public servants the right to decide what is good for the people to know and what is not good for them to know. We insist on remaining informed so that we may retain control over the instruments of government we have created.
